Once your CIS card application form is complete
you are required by the Inland Revenue to submit
it in person. You should take your form with a
passport sized photo to your local tax office.
Take 2 x proof of identity, one showing a photo
such as a passport and another showing your address,
such as an utility bill. If your NI number is
not printed on the form you will need to take
additional proof of this.
